Terms & Conditions

  • Veterinary physiotherapy will only be provided to animals after completion of a veterinary consent/referral form by the veterinarian. Veterinary referral certifies that your vet has given permission for me to perform veterinary physiotherapy on the stated animal. ​

  • Upon first appointment you, as the owner, will be required to complete an owner consent form, certifying that you give permission for me to provide physiotherapy for your animal. 

  • Payment is to be made in full on the day of treatment by either cash, cheque or bank transfer. If no payment is received 5 days after treatment, the service will cease until payment has been received. 

  • Cancellation within 24 hours of appointment will incur a 50% charge of the total cost of the appointment. 

  • No animal that is found to have sickness, diarrhoea or flea infestation will be treated. 

  • Charlotte Duke Veterinary Physiotherapy strives to provide an excellent service and respond to any issues promptly, and ask that in such an event, you contact me at your earliest convenience.
